Use this form to request a reprint of lost or damaged ABKC documents. Incomplete applications will cause delays. A $5 service fee will be withheld for refunds.

Combine Shipment
If you have applications from multiple forms and wish to combine into a single shipment, use this option. USPS First Class Mail & FedEx available. All orders must be submitted on the same day within the same hour. 5 item maximum. This button must be selected on ALL apps you wish to combine. If one or more applications have processing errors, items will be shipped separately. If FedEx services are selected, only 1 FedEx payment is required for shipment.
Incomplete applications will cause delays. If refunded, Processing & Admin Fee ($7.00) will be withheld.
